What kind of forms required to fill up for TPS?

Also for I 765, what will be the category for H1B on question 16.

You are filing TPS it doesnt matter what your status is...write a(12) or c(19)..Thats what people are writing if you have time to read other older posts related to TPS.

Anybody know if TPS beneficiary need to enroll in Obamacare? If not what to do when filing taxes? Will there be any penalty?

I think Obama care is irrevelant to status / citizen ship. As long as you can file taxes you have to have insurance.

16. for the one who is applying for Temporary Protected Status (TPS)--

Just TPS not EAD

is it (a)(12) ) Or (c)(19). ?

Both will work perfectly fine. My spouse put a(12) and she was approved couple of months ago.
